Are you feeling low or depressed? PHQ-9

The PHQ-9 is a simple to use, nine item questionnaire that measures response to treatment and the severity of DEPRESSION.

The questionnaire asks you how often have you been bothered by any of the following problems, over the last two weeks. Not at all Several days More than half the days Nearly every day
Little interest or pleasure in doing things
Feeling down, depressed, or hopeless
Trouble falling or staying asleep, or sleeping too much
Feeling tired or having little energy
Poor appetite or overeating
Feeling bad about yourself — or that you are a failure or have let yourself or your family down
Trouble concentrating on things, such as reading the newspaper or watching television
Moving or speaking so slowly that other people could have noticed? Or the opposite - being so fidgety or restless that you have been moving around a lot more than usual
Thoughts that you would be better off dead or of hurting yourself in some way

PHQ-9 Score:

This suggests you do not have depression as measured by this questionnaire. However, you may still be struggling and could still benefit from our help.

PHQ-9 Score:

This suggests you have mild depression and could benefit from our help, so get in touch.

PHQ-9 Score:

This suggests you have moderate depression and could benefit from our help, so get in touch.

PHQ-9 Score:

This suggests you have moderate-severe depression and could benefit from our help. You may also wish to contact your GP to discuss your problems.

PHQ-9 Score:

This suggests you have severe depression and are advised to contact your GP to discuss your difficulties. You could benefit from our service so get in touch.

You are indicating that you have thoughts that you would be better off dead or of hurting yourself in some way. We would strongly recommend that you get in touch with your GP to discuss this.

The GAD-7 is a simple to use, seven item questionnaire that measures levels of ANXIETY.

Over the last 2 weeks, how often have you been bothered by any of the following problems? Not at all Several days More than half the days Nearly every day
Feeling nervous, anxious or on edge
Not being able to stop or control worrying
Worrying too much about different things
Trouble relaxing
Being so restless that it is hard to sit still
Becoming easily annoyed or irritable
Feeling afraid as if something awful might happen

GAD-7 Score:

This suggests you do not have anxiety as measured by this questionnaire. However, you may still be struggling and could still benefit from our help.
